I have had this file on my computer for almost 5 years and now am finally posting it in January 2021. This was emailed to me by Alex Panzer in March of 2016. Thanks Alex! The PDF scan is somewhat pixelated from the scan process but is pretty good over all. This is the first CSP I have seen with mulitple counties in it. This document is in PDF format.
